Disclaimer and Liability Release

I acknowledge that Square-Immune is not a drug and has not been approved by the U.S. Food and Drug Administration to treat or prevent any disease.

I acknowledge that Square-Immune may cause a rash and redness at the application spot when it is applied to skin.  The pain and itching of the rash is typically mild and typically lasts less than 7 days.  The redness may persist for two months over even more.  Therefore it is recommended that Square-Immune be applied only to the arm or other relatively hidden places on your body and not to your face or lips.  

I acknowledge that In rare cases, persons have had a broader allergic reaction to the ingredients of Square-Immune and have had swelling in the face and other parts of the body distant from the application site and a rash that extends well beyond the application site and persists.

I understand that Square-Immune is for topical use only and should never be ingested.  

I hereby release Gamma Three, LLC, and Squarex Pharmaceutical Corporation and Hugh McTavish from all liability arising from my use of Square-Immune.
